2019-2020 Survey Results
We are proud to acknowledge the incredible effort and work of both our Anchin and Benderson teams during the survey process this year! Anchin Assisted Living and Memory Care received a zero-deficiency survey, and the state survey team acknowledged several individual employees for their extraordinary effort. Benderson Skilled Nursing & Rehabilitation received a near-perfect score on its survey. These results solidify our 5-star Overall Rating under the Centers for Medicare and Medicaid. In addition to this wonderful overall rating, the team received 4-stars in both Quality Measures and Staffing.
The Senior Living industry remains one of the highest regulated entities in the United States. Under both state and federal regulations, our teams are expected to undergo a regular survey period for all levels of care at the Anchin-Benderson building. During this period surveyors from AHCA (Agency for Health Care Administration) meticulously examine every aspect of our buildings to check for violations or what is more commonly known as deficiencies. Surveyors examine the physical attributes of the building, how care is distributed, our filing system, even as far as testing the condition of the paint on the walls, checking to ensure each element is at the highest possible standard of quality. The ultimate goal is to ensure that every organization in the state of Florida and throughout the United States is protecting and serving their residents.
Providigm QAPI Accreditation
Our team at Benderson Skilled Nursing & Rehabilitation is excited to announce our successful completion of the Providigm QAPI Accreditation process.
Providigm is an independent accrediting organization that works across the globe with healthcare facilities to improve quality of care and recognize outstanding organizations.
This new accreditation recognizes that we are continually assessing residents’ quality of life (managing pain, maintaining dignity, respecting resident choice) and quality of care issues. Providigm’s QAPI Accreditation Standards require that we are assessing our quality against the full federal regulation at an ongoing rate, encompassing a substantial proportion of our residents, and correcting identified issues.

Patient Care at Its Finest
Benderson Skilled Nursing on the Aviva campus offers rehab services in a nurturing environment, with the goal of getting residents back on their feet when they need it most. Providing a solid continuum of care is important to us, and our comprehensive physical therapy, occupational therapy, and speech therapy services utilize skilled nursing professionals who are available 24-hours a day. Residents can sleep easy knowing that Aviva’s rehabilitation services are synonymous with low return-to-hospital rates and high return-to-home rates.
After demonstrating the level of care we give to patients, Benderson’s Skilled Nursing and Rehab services received a QAPI accreditation. Providgm’s QAPI Accreditation is based on four standards that evaluate care and life for nursing home residents. To receive accreditation, communities must meet objectives for comprehensive and continuous care, coverage standards, and corrective action. Each of the Four C’s is measured quarterly for a 12-month period, comprised of resident observations, staff and resident interviews, family interviews and comprehensive evaluation of records.
The results are in, and both Aviva’s Anchin Assisted Living and Memory Care and Benderson Skilled Nursing and Rehabilitation received zero-deficiency surveys for their exceptional care programs. Each year, The Centers for Medicare and Medicaid Services (CMS) performs a rigorous set of examinations that take a comprehensive look at essential nursing care elements like quality of care, quality of service, patient rights, and more. It’s rare for a senior living community to receive a deficiency-free survey, meaning that we’re providing the best care to our residents possible.
For the past 25 years, we’ve dedicated our lives to improving the wellness of our residents.
